Using Outcomes-Based Financing to Invest in Social Determinants of Health

An overview of outcomes-based financing for policymakers, impact funders, and Medicaid managed care organizations (MCOs) that have an interest in leveraging this model to invest in preventive health interventions that target the social determinants of health (SDOH). Our specific focus in this brief is using outcomes-based financing for interventions targeted at individuals covered by Medicaid – a program that covers over 74 million Americans and serves many of the individuals who can benefit most from interventions that address SDOH.
